I grant consent to Arkansas Christian Academy Preschool, or it's representative, to request information pertaining to my child from previous childcare providers/references. Such reference checks are necessary for the admissions process and for the purpose of enrollment, if an offer of acceptance is extended. Furthermore, I waive my right to access confidential information contained in my child's admission file. I certify that I understand Arkansas Christian Academy is not required to disclose any reasons for which my student may not be selected for admittance.
I affirm that all of the information I provide Arkansas Christian Academy Preschool is correct, complete, and honestly presented. I understand that all registration payments are non-refundable/non-transferable. I understand that withholding or misrepresenting information may jeopardize my child's admission.
